Supporting adults with neurological
conditions and other complex needs

In association with our Therapy Team from Neural Pathways,
Chase Park Rehabilitation Centre provides facilities to
support health promotion, the development of every day skills
and stimulation of the senses, whilst also offering
opportunities for relaxation and tranquility. We offer short
and long term placements including continuing-care and high
dependency nursing for people aged 18 and over.
Key Concepts...
- A person centred approach where family and friends are
an integral part of the team.
- To facilitate the client to self-care, and to empower
them to assert control over their own lives.
- We aim to enable clients in their everyday lives and
provide
opportunities for independence and lifestyle
choice.
- We believe that rehabilitation and long term
continuing-care packages need to produce meaningful and
sustainable outcomes and we always strive towards
individually tailored goals.
- To facilitate the transition to community living.
We have shared values and a vision to enable our
clients to:
- Receive a holistic service
- Provide social opportunities allowing inclusion and
participation in community activities
- Make informed choices
- Progress their abilities and fulfil their potential
- Be treated with respect and dignity, and have a valued
social role
