In association with our Therapy Team from Neural Pathways, Chase Park Rehabilitation Centre provides facilities to support health promotion, the development of every day skills and stimulation of the senses, whilst also offering opportunities for relaxation and tranquility. We offer short and long term placements including continuing-care and high dependency nursing for people aged 18 and over.

Admission Procedure & Service Pathway

Pre-admission assessment at client’s current place of residence
Key Worker, Named Nurse and Therapy Co-ordinatormeet client on day of admission
Comprehensive rehabilitation, care and treatment programmes devisedbased on full assessment of individual strengths and needs
Individual programmes are coordinated at weeklymultidisciplinary team meeting, with plans followed up through the week.This promotes outstanding communication throughout the team
3-monthly reviews, involving client, family and multi-disciplinary team; accessible written report produced to consolidate issues discussed for continuous development
Where discharge planning is an integral part of a clients programme potential discharge options are considered from the date of admission
Discharge processes are well planned and supported to ensure sustainability of placement

Therapy and Rehabilitation from Neural Pathways

Bespoke therapy packages are available in addition to our standard service. This service is provided in partnership with Neural Pathways a locally based specialist team of therapists who are leaders in the field of therapy and rehabilitation with a reputation for exceptional quality, efficient service and sustainable outcomes.

Individual therapy opportunities can be extended into a wide range of activities throughout the day within Millfield House and the wider community.

Therapy form Neural Pathways can be arranged prior to admission as part of the resident’s total package, or at any time via self or family referral - please ask for details.

All therapies require an initial assessment with the service tailored to a very individual level led by the choices and needs of each resident.

Opportunities Team

Chase Park Rehabilitation Centre has developed a team of dedicated individuals whose role is to promote, facilitate and enable social opportunities and inclusion for our clients.

Our Opportunities Team are directed by our therapy team. This ensures therapeutic goals and lifestyle choices can be extended into a wide range of activities throughout the day within Chase Park and the wider community.

The Opportunities Team bridges the gap between therapy and care and supports our clients in transferring the skills acquired into their lifestyles. The service is tailored to a very individual level led by the lifestyle choices each client wishes to make.

The opportunities team make full use of the facilities offered by the Newcastle-Gateshead conurbation which provides a huge range of facilities such as the Metro Centre, St James' Park, The Sound Room, riverside walks and many other attractions. The area is surrounded by beautiful countryside and the east coast is less than 1-hour away. More locally the adjacent Chase Park offers an easily accessed tranquil getaway. The Centre itself along with The Healthclub and our own accessible transport provides many choices for activities both relaxing and more challenging. Regular holidays and days out are facilitated by the opportunities team, particularly during the summer months.

A lively residents' forum exists and this is facilitated by members of the opportunities team, with regular meetings and proactive consultation.
